Club players will need the following for the 2011-12 season registration:
A copy of their birth certificate
A current photo (1 inch x 2 inch)
A copy of your health insurance card and the following completed forms:
-CFC Fee Commitment (download your copy here)
-IYSA Medical release (download your copy here)
